This is a free,online textbook that is continually being updated.  It is comprised of a variety of articles dating from 1995 to the present.  The authors refer to the material as a "Grand Rounds Archive." The materials can be read directly online or downloaded as pdfs.  In some cases there are slides and videos that can also be downloaded.
